Valsalva is the forceful expiration of air with the patient's mouth and nose held shut.  When performed in this fashion, air will be forced into the middle ear cavity and help treat eustachian tube dysfunction.  This patient has a myringostapediopexy.  By forcing air in the middle ear, he is able to blow air into the middle ear which will help to prevent progression of the eardrum's breakdown.
